ELDERLY NUTRITION PROGRAMS

Senior Dining:
Senior Dining meals are available to persons 60 or older and spouses of an active senior dining participant regardless of age.

Senior Dining provides more than a hot, nutritious noon meal for elderly persons. Meals are enjoyed in a warm, family-like atmosphere where opportunities for socialization and friendship can be found.

Donations are accepted for the meals to help provide funding for the nutrition program. The suggested donation per meal is $3.00. The program is available to all seniors regardless of income.

Senior Dining locations are:

Eastman, Salem United Methodist Fellowship Hall, Monday through Friday, serving at 11:00. For a reservation call 608-874-4364.

Prairie du Chien, Prairie Maison Nursing Home, (720 South Fremont Rd) Monday through Friday, serving 11:15. For more information call 608-326-0235. To make a reservation call 608-326-3224.

Soldiers Grove, TazZee’s Restaurant, Monday, Wednesday, Thursday & Friday, serving at 11:30. For a reservation call 608-624-3295.

Home Delivered Meals:
Home Delivered Meals are intended for the elderly person who is homebound and unable to attend senior dining facilities; unable to shop and/or cook or has no one available to perform these services. Home Delivered meals are delivered Monday through Friday in Prairie du Chien and Monday, Wednesday, Thursday and Fridays on the northern routes

Special diets are available upon request. Each person applying for home delivered meals must be assessed to determine the individual’s need for service. An initial phone interview is conducted to determine eligibility.

Donations are accepted for the meals to help provide funding for these services. The suggested donation is $3.50 per meal. No one is denied meals because of their inability to pay.

Weather and road conditions vary widely throughout the region and we occasionally cancel programs for the safety of drivers and clients. In the case of inclement weather, Aging & Disability Resource Center cancellations are always announced on WQPC/WPRE radio 94.3 FM. on the day. If you are in doubt about whether home delivered meals or bus route will run, please contact our office.

Nutrition Intervention Program:
A registered dietitian is available to provide guidance to seniors on nutrition and health information. Donations are accepted, but no one will be denied services if a person does not make a donation.

Crawford County Nutrition Intervention Program Grievance Procedure:

Our goal is to provide you with excellent customer service. 

If you have a complaint concerning the Crawford County Nutrition Program, you are encouraged to first discuss the problem with those individuals involved. Have you discussed your concern with the involved staff person(s)?  If not or you are uncomfortable speaking to the involved staff person(s), you may speak with the Director.

1) Contact or initiate a written formal complaint to:

A complaint form is available for this purpose.  Your complaint must be received by the ADRC Director within (30) thirty days following the incident.  A written review of your complaint will be sent to you within (14) fourteen days of its receipt. 

2) If your complaint still is not resolved, ask the ADRC Director to forward
    your Complaint to the ADRC Nutrition Advisory Board Chairman:

    Or, you may forward it yourself.  If you wish to add information, you can.  A written review of your complaint will be sent to you within (14) fourteen days of its receipt.

Confidentiality of all participant information relative to grievances shall be maintained.  You may pursue your grievance with the Greater Wisconsin Agency on Aging Resources, Inc.1414 MacArthur Rd, Madison, WI 53714.

A Benefit Specialist is a person trained to help persons 60 years old and older who are having a problem with their private or government benefits. Benefit Specialists receive on-going training and are monitored by attorneys knowledgeable in elder law from the Elder Law Center of the Coalition of Wisconsin Aging Groups.

A Benefit Specialist can help you with:
• Medicare and Medicare Supplemental Insurance
• Medicare Part D – Prescription Drug Program
• Social Security & Supplemental Security Income (SSI)

DISABILITY BENEFIT SPECIALIST

A Disability Benefit Specialist is a person trained to help persons 59 years old and younger who are having problems with their private or government benefits.  Disability Benefit Specialists receive on-going training and are monitored by attorneys knowledgeable in elder law from the Elder Law Center of the Coalition of Wisconsin Aging Groups.

A Disability Benefit Specialist can help you with:

•  Provide confidential assistance to adults ages 18 to 59 with physical
   disabilities, developmental disabilities, mental health issues, and/or
   substance use disorders.
•  Provide current, accurate information about public and private benefit
    programs.
•  Provide guidance regarding eligibility for various programs and benefits.
•  Provide assistance to people in applying for SSI/SSDI or appealing a
    denial of benefits.

   INFORMATION AND ASSISTANCE SPECIALIST


Our friendly, professional staff can staff can provide objective information and assistance about programs and services throughout the area as well as public benefits that may be available to older adults, people with disabilities, and adults challenged by mental health and/or substance abuse disorders.  Individuals can receive information specifically tailored to the situation.

Our staff will discuss services and options available for you or a loved one.  You can stop in and see us or we can schedule a visit at a place that is convenient for you.

TRANSPORTATION

Five Senior Traveler vehicles provide door-to-door transportation to persons 60  years and older to senior dining centers and for special requested trips on a “first come, first serve” basis.

Disabled residents and persons 60 years of age and older may ride the any of the Senior Traveler for shopping or non-emergency medical appointments on “a seat available” basis.

Crawford County Service Areas:
Senior Traveler I serves the northern part of Crawford County.  Senior Traveler travels to the Soldiers Grove meal site each weekday except for Tuesdays.  On Tuesdays it travels to Viroqua, La Crosse, or Prairie du Chien for medical/shopping/personal business.

A handicapped-accessible mini-van is also available in northern Crawford County to transport disabled and persons 60 years of age and older to non-emergency medical appointments and shopping.  It also transports passengers on the Kickapoo Transit Service on Thursdays.  The Kickapoo Transit Service is the Gays Mills to Viroqua loop operating on Mondays and Thursdays between Crawford and Vernon Counties.

Senior Traveler II serves southern half of Crawford County.  Senior Traveler II travels to the Eastman meal site each Monday and Wednesday, while Tuesdays and Thursdays are reserved for non-emergency medical/shopping/personal business.  Special use trips maybe schedule with approval of the ADRC.

Senior Traveler III provides services throughout Prairie du Chien.  It travels to the Prairie du Chien meal site daily and is also used for non-emergency medical/shopping/personal business.

A handicapped-accessible mini-van is also available in Prairie du Chein to transport disabled and persons 60 years of age and older to non-emergency medical appointments/shopping/personal business.

The Crawford County ADRC does not provide transportation for Medical Assistance Members.  Members must call LogistiCare at 1-866-907-1493 to schedule three days prior to appointments.

OTHER SERVICES

National Family Caregiver Support Program:

The National Family Caregiver Support Program is designed to help family caregivers meet the needs of their dependent loved ones.  You may be eligible if you are a caregiver of a person 60 years of age or older or are a caregiver of a person who have been diagnosed with Alzheimer’s or other dementia that are below the age of 60 years of age.  Grandparents or older individuals that are relative caregivers of children under 19 years of age; Grandparent or relative caregivers (must be 55 years old), providing care for adult children with a disability who are between 19 & 59 years of age.

Caregiver Services include:
• Respite care in-home or institutional
• Information and assistance to caregivers
• In-home supplemental services such as chore, personal care and assisted
  transportation
• Individual counseling and skills training for caregivers.

The Triple "C" Crawford County Caregivers Support Group:

The Triple "C" Support Group offers people dealing with similar issues an opportunity to develop a mutual support system.  The Aging & Disability Resource Center (ADRC) currently facilitates care-giver support groups in Prairie du Chien.  Meetings will be held the 2nd Tuesday of the month from 5 to 6:30 p.m. at the Peoples State Bank (main office) in Prairie du Chien.  Please contact Teresa Gleason at 608-326-0235 with any questions.

Crawford County Communique:

The Communique is a monthly newsletter published for seniors. The newsletter includes senior dining and home delivered meals menus, Senior Traveler schedules, blood pressure, Benefit Specialist schedules and many informational articles on senior issues.

The Communique is distributed throughout Crawford County at area grocery stores, banks, restaurants, elderly housing units, and at all senior dining centers.

AARP - Tax Aid:

The AARP Tax-Aide program was created to help low and middle income taxpayer prepare and file their income tax returns with the IRS, free of charge.  Special consideration is given to individuals 60 years of age or older.

From mid-February through March trained and certified volunteers will prepare your federal and state tax returns at locations around Crawford County.  Your return will be filed electronically, allowing you to receive your refund faster.

Check back mid-February 2013 for the schedule filing 2012 taxes.

                            HEALTH & WELLNESS

Stepping On Program:

The Stepping On Program offers older people a way of reducing falls and at the same time increasing self confidence in situations where they are at risk of falling.  The program covers a range of issues, including falls and risk, strength and balance exercises, home hazards, safe footwear, vision and falls, safety in public places, community mobility, coping after a fall and understanding how to initiate a medication review.

The program is over a seven week period and is usually twice a year.  Contact the Aging & Disability Resource Center Office 608-326-0235 for more information.

Strong Women, Strong Bones Program:

The Strong Women, Strong Bones Program is a weekly program for men and women of all ages.  Each class includes progressive weight training, flexibility, balance activities, and nutrition education.

Research shows that a program of strength training
*  Improves bone density
*  Reduces falls
*  Improves arthritis symptoms
*  Increases flexibility
*  Increases strength

Benefits Mental Health
Improves mood & attitude
*  Decreases depression
*  Allows you to sleep more soundly
*  Increases energy level
*  Provides the opportunity to socialize with others & build a new circle of friends

These exercises are safe and effective for women and men of all ages, including those who are not in perfect health.

People with health concerns such as arthritis, heart disease, osteoporosis, diabetes, obesity, and back pain often benefit the most from an exercise program that includes lifting weights a few times each week.

Emergency Preparedness:

Your preparedness before an emergency occurs is a the key to surviving an emergency.  Please prepare now.

Emergencies can happen anytime and often they are unexpected.  We often think of emergencies being weather related, such as tornados, flooding, blizzards, etc., but they can be crisis related, too. 

A crisis emergency would include a fire in your home/apartment building a pandemic flu outbreak or a chemical spill in the community.  Regardless of the emergency situation, it is very important to be prepared ahead of time.
